#14 Domino's
In 2009 a couple of Domino's employees made a video of the disturbing things that allegedly went on in the kitchen. In the video, they stuck cheese up their nose and put salami in questionable places before serving the ingredients up in a sandwich. This is not how Domino's operates and the pair were fired immediately, but we still think this is terrifyingly gross!

Carl's Jr/Hardee's
Don't get us wrong, Carl's Jr. is delicious! What's not so delicious is that they averaged 118 critical violations per 100 inspections. We're not even sure how something so terrible happens but a Western Bacon Cheeseburger sure sounds good right about now.

Jack In The Box
Jack In The Box is said to rank above most of the other food chains in overall health code inspections. That's great because their tacos are fantastic, but it doesn't make them perfect. The chain averages 45% in critical violations. That's just less than half... How comforting.

Arby's
It's good mood food! Maybe it's their ability to pull of a slogan with so many O's in it but they have averaged 115 health code violations for every 100 inspections. Most of which are temperature violations involving leaving their food out for hours on end. We're wondering if nearly-spoiled food contributes to a good mood.

KFC
Everyone loves a bucket of chicken! But their health code say otherwise. They have a high report of customers that say they have gotten food poisoning from this chain. In Chandler, Arizona one of the locations received 8 consecutive violations for mold, grime and selling outdated foods.

Dairy Queen
They had a high rate of health code violations overall but mainly for their food production. Their main offense was in the touching of raw meet and cooked food without washing hands or changing gloves between. Their sweet, frozen treats are in great standing as far as health and deliciousness go, so who wants to go get a Blizzard?

Claim Jumper
In Mission Viejo, California a man was enjoying a bowl of french onion soup only to discover part of what could only be identified as a "used condom". The manager later claimed it was part of a rubber glove.

McDonald's
C'mon, did you really think we'd leave Ronald out of this? On average, McDonald's ranks as the highest offender in health code violations per restaurant, mostly from lack of cleanliness from the employees. If that wasn't enough for you, in 2009, a customer in the Seattle area allegedly found mucus in her iced tea! Gross!

Burger King
In 2008 a Burger King employee decided her needed to take a bath. Rather than going home like normal people, he hoped into the kitchen sink at work. This story made headlines and resulted in some harsh consequences for the "Burger King bather" but what terrified us more was their contamination and holding temperature violations. While you're not going to see employees bathing there, you may still come in contact with some contaminated food... Bon Appetit, everybody!

Subway
Everyone wants to "eat fresh" but those visiting some Subway restaurants may get more than they bargained for. The chain has an average of 98 violations out of every 100 inspections. This is not as bad as some other chains but it's still not perfect.

Wendy's
This chain continues to improve in their health inspections. They only had 84 violations for the 100 inspections, which ranks them as one of the better fast food kitchens.

Cracker Barrel
Cracker Barrel had to recall hamburgers from 313 restaurants after a customer cut her mouth on metal embedded in a meat patty. Now, that's terrifying!

Taco John's
In 2006, this chain was responsible for a large E. coli outbreak through their locations in Texas, North Dakota and Iowa. It was traced back to the growing fields in California where the chain received produce. The company took a hit in sales for a while but have since bounced back.

Chi-Chi's
No one has to worry about the problems in Chi-Chi's kitchen anymore but in 2003, this chain was responsible for the largest hepatitis A outbreak in U.S. history with nearly 700 victims in the Pittsburgh area. The company went bankrupt a month later, settled all of their lawsuits by the next year and the company's name has since been bought by Hormel Foods to sell salsa and tortilla chips. We think this one is both terrifying and sad.
